Application Process

​

Step 1: 

  • You must make an appointment and meet with the Grants Manager to discuss your proposal.  This meeting should occur at least 30 days prior to the grant deadline.

​

Step 2:

  • If after meeting with the Grants Manager, it is determined that your proposal is eligible for funding, you will receive the CMC Community Health Foundation Grant Application and Outputs and Outcomes Expectations Report.

Applicants who submit their grants early may request feedback from our Grants Manager prior to final submission.

Application Deadlines

There are two application windows per year.

​

  • Application Window #1: January 1st - May 1st 

  • Application Window #2: July 1st - November 1st

*ALL APPLICANTS MUST contact the Grants Manager prior to submitting an application. Applications must be emailed by 5pm on the day the application window closes.
